Acceleration/Deceleration during Space Travel

First question: Perhaps its just sci-fi, perhaps not, but why would spaceships going millions of miles away need to constantly keep their engines running?



Without gravity, they should be able to just reach point the ship in the right direction, and engage the engines until desired speed, right?



Second question: obviously a space ship going to Mars or further would not want to continuously engage their engines, as they would need to eventually slow down and that would be very difficult to do without tearing apart their human passengers, unless they did slowly & progressively over a very distance, am I right?



So, what is the optimum speed a spaceship would want to reach on the way to other planets/solar systems?
