Howare celebrity endorsements and forewords arranged in the publishing industry?

Let's use this book (very good btw) as an example. It has an endorsement by Richard Dawkins and this newer edition apparently has a foreword by Christopher Hitchens, both of whom can reasonably be considered celebrities, especially to the likely audience of that book.

What I wonder is, how do such things work in the publishing industry? Did someone (Victor Stenger? His publisher?) pay Dawkins for the endorsement, or did he endorse it on his own accord? And Hitchens' foreword, does he recieve a percentage of the revenues from the book? Or how does it work?
