Hi there- How does everyone feel about Home birth? Water Birth?
I think home deliveries done following ACOG guidelines are acceptable but I also feel you should inform the mother of the risks related to evidence based practice. Where I am located that doesn't always happen because there are different levels of midwifery service, those that are nurses and masters prepared and those that are lay midwives or direct entry midwives ( DEM).

I think using the hot tub, while monitoring the temp of the water, as a method of pain control is fine in the early stages of labor. I don't think it's a good idea to opt for a water delivery simply because transition is dependent on that first breath. The "diving reflex" doesn't necessarily over ride the physiological processes prompting that first breath. This can predispose the infant to pneumonia and other complications after delivery.
The original thought process behind water birth was that it was more natural, but I say that is a misconception. No primate goes to a river, lake, or ocean to deliver. For that matter, with the exception of dolphins and wales, no mammal seeks water to give birth. The aquatic ape theory is also an unfounded theory of origin more to do with bipedalism than anything to do with labor and delivery. If this were truly how we were intended to deliver our young I would think you would see it more often in nature, but we don't.
